How to Refer

• Inquiry: Complete our Provider Referral Form or have the client contact us directly.

• Consent & Disclosure: If submitting an inquiry on a client's behalf, please ensure:

• The client has authorized Mind Mosaic to contact them regarding this referral.

• The client is aware that we are a private-pay practice.

• Collaboration: We highly encourage providing a Release of Information (ROI) alongside the referral to allow for seamless clinical collaboration.

• Intake: Once we receive the inquiry, we will reach out to the client to coordinate scheduling and services.
