Accessing Your Client Portal

Accessing the TherapyNotes Client Portal

You can reach the client portal at:  https://www.therapyportal.com/p/mindmosaic/

  • Use the login credentials given to you in your Welcome Email. 

  • Please contact Jen at help@mindmosaicpsych.com if you did not receive or have misplaced your login information.

  • Inside your portal, you will have access to any upcoming appointment information from the main page. 

  • You will also have access to all forms, documents, and billing records by clicking DOCUMENTS (upper right corner).

From here, you will find two tabs: 

1) PAPERWORK AND FORMS

This is where you will find any documents or forms that have been assigned for you to complete or review. For example:

  1. Payment Authorization

  2. Intake documents

  3. Privacy Practices

  4. *Release of information

  5. A copy of your psychological evaluation 

*Release of Information: Please Note, a separate release form is required for each individual or organization that you are allowing Mind Mosaic Psychology to send and/or receive information from.

2) CLIENT RECORDS

Here you will find copies of all completed forms for your records.

*Release of Information: A blank Release of Information is always available in your client portal. Completed forms are available from the Client Records tab. To submit additional release forms, please return to the main DOCUMENTS page, select Paperwork and Forms and click Release of Information.

Schedule an Appointment

We accept your online inquiries 7 days a week and have specific office hours for in-person engagements. Outside of normal office hours you can receive immediate assistance at a local Urgent Care facility or, if necessary, please dial 911 for emergency services.
