Insurance
We do not currently accept insurance; however, we will provide you with a detailed receipt (superbill) that you may submit to your insurance company to seek reimbursement.Mind Mosaic Psychology is Out-Of-Network for all insurance plans. 
Reimbursify
Our clinicians are also enrolled with Reimbursify; which is a secure, HIPAA‑compliant mobile and web‑based service that helps clients easily file out‑of‑network health insurance reimbursement claims for mental health and other healthcare services. It simplifies what is often a confusing and time‑consuming process by allowing clients (or your practice on their behalf) to submit and track claims electronically, helping them access the insurance benefits they’re entitled to with less paperwork and stress

Cancellation Policy
For consultations, cancellations within 48 hours of your appointment may result in a charge for the full time reserved.
For testing, cancellations or appointment rescheduling within 10 business days may result in a charge for the full time reserved.
When significant illness, and/or accidents that require hospitalization occur, and rescheduling is not possible due to the extent of medical injury, then our office has the discretion to determine a refund.
This is with the understanding that we are not able to reserve that time with others who are interested in meeting during that same period. You are asked to email the office in addition to calling the main office, to ensure that we can coordinate schedules and make adjustments as needed.

Fees: Listed fees are based on 6 hours of testing. If more or less time will be required, this will be discussed with you and your bill adjusted accordingly.

Includes:
Testing (in-person, 1-2 hour appointment): This includes a brief parent meeting as well as administration and scoring of a standardized cognitive assessment *Appointment time may vary depending on how your child tests.
(If applicable) Completion of program specific forms and secure sharing of results with the school.

Payment Due: Full balance due at the time of booking Please note, these evaluations are not typically reimbursed by insurance

Testing (in-person, 1 to 2 hour appointment): This includes a brief parent interview as well as administration and scoring of assessments as required by the child’s school program.
Electronic rating forms (if required by a child’s school program), scoring, interpreting, and writing results.

Examples:
Your provider may be able to attend virtual meetings (e.g., IEPs, 504s, Transition Planning, meetings) if requested pending availability.
These meetings are billed as virtual consultations.

Sarah Smead, LPA: $250/ hour
Taylor Shirkey, LPA: $230/ hour
Payment Due: The first hour is due at the time of booking. Should more than one hour be needed the remaining balance will be due at the completion of the service.

We accept your online inquiries 7 days a week and have specific office hours for in-person engagements. Outside of normal office hours you can receive immediate assistance at a local Urgent Care facility or, if necessary, please dial 911 for emergency services
