Services and Fees

We strive to be transparent and thoughtful about our fees so families and clients can make informed decisions about care. Evaluations and other psychological services are individualized for every client. For this reason, services, timelines, and exact fees will always be reviewed with you before scheduling (also known as a Good Faith Estimate). The information below is intended to provide an overview of our service offerings.

Evaluation Packages

  • Including:

    • Intake session (60-90 minutes)

    • Testing (in person, 4-6 hours)

    • Electronic rating forms, collateral information and data, scoring, interpreting, and writing results

    • Feedback with client and/or family/guardians

    Clinician Rates:

    • Sarah Smead, LPA: $3400

    • Taylor Shirkey, LPA: $3100

    Fees: Listed fees are based on 6 hours of testing. If more or less time will be required, this will be discussed with you and your bill adjusted accordingly. 

  • Includes:

    • Testing (in-person, 1-2 hour appointment): This includes a brief parent meeting as well as administration and scoring of a standardized cognitive assessment *Appointment time may vary depending on how your child tests. 

    • (If applicable) Completion of program specific forms and secure sharing of results with the school.

    • Data, scoring, interpreting, and writing results

    Clinician Rates:

    • Sarah Smead, LPA: $475

    • Taylor Shirkey, LPA: $ 450

    Payment Due: Full balance due at the time of booking Please note, these evaluations are not typically reimbursed by insurance

  • Includes:

    • Testing (in-person, 1 to 2 hour appointment): This includes a brief parent interview as well as administration and scoring of assessments as required by the child’s school program.

    • Electronic rating forms (if required by a child’s school program), scoring, interpreting, and writing results.


    Clinician Rates:

    • Sarah Smead, LPA: $575

    • Taylor Shirkey, LPA: $550

    Payment Due: Full balance due at the time of booking Please note, these evaluations are not typically reimbursed by insurance

    Please Note: A Superbill is not provided for IEE services as these are ineligible for insurance reimbursement.

Abbreviated Assessments & Individual Services

Unless otherwise noted, payment for services listed below is due at the time of booking. 

  • Example Consultation Topics:

    • Follow-up appointments after feedback to clarify diagnoses, results, or recommendations

    • Parent or caregiver consultation and support

    • Clinical guidance related to understanding needs and next steps

    *These services are usually billed as private pay and are not submitted to insurance.

    • $125 (30 min)

    • $250 (60 min)

    • $375 (90 min)

    Taylor Shirkey, LPA

    • $115 (30 min)

    • $230 (60 min)

    • $350 (90 min)

  • Examples:

    • Testing less than 4 hours or if  additional testing is required beyond the 6 hours included in the comprehensive price listed above

    • Hours are calculated based on in-person testing time. 

    • Fees include the cost of all electronic rating forms, test administration, scoring, interpretation, and report writing

    Clinician Rates:

    • Sarah Smead, LPA: $475/ hour

    • Taylor Shirkey, LPA: $450/ hour

  • Examples:

    • Your provider may be able to attend virtual meetings (e.g., IEPs, 504s, Transition Planning, meetings)  if requested pending availability.  

    • These meetings are billed as virtual consultations. 

    Clinician Rates:

    • Sarah Smead, LPA: $250/ hour

    • Taylor Shirkey, LPA: $230/ hour

    Payment Due: The first hour is due at the time of booking. Should more than one hour be needed the remaining balance will be due at the completion of the service. 

  • Includes: Short forms (1-2 pages) outside of the standard evaluation process 

    Fees:

    $30 - Flat Rate

    Payment Due: Prior to form completion.

We operate as a private-pay practice because it allows us to provide a level of care that standard insurance models often cannot support. Lower-cost, insurance-based assessments often rely on standardized protocols, limited testing time, and generic recommendations. While these may provide a label, they frequently leave clients with more questions—and more expenses—down the road.

By working independently of these constraints, we are able to offer the highest quality clinical services that are:

  • Truly Individualized: We design each evaluation around your specific questions, selecting the exact tools and spending the time needed—without external limits on testing duration or report length.

  • Timely and Accessible: Avoiding administrative approvals and delays lets us schedule more quickly and spend the time each evaluation actually requires.

  • Objective and Insightful: We provide high-level clinical interpretation to ensure your report serves as a functional tool for schools and workplaces.

Payment Options & Navigating Reimbursement

  • Zelle

  • Most families also find that our services are eligible for HSA/FSA spending.

Insurance Reimbursement

While we do not bill insurance directly, many plans offer out-of-network benefits that may reimburse a portion of your costs.

To help, we will provide a  Superbill when you complete your evaluation process.

A Superbill is an itemized receipt with clinical codes that you can submit to your insurance provider to request reimbursement.

Education Student Accounts (ESA+) Grants

The Education Student Accounts (ESA+) Program has reviewed our providers and covers grant-funding for testing. Please look at their website for guidelines for applying for this grant to use for testing and/or school options.

Our goal is a definitive evaluation that reduces future uncertainty and provides a clear, functional path forward.

  • Because we don’t believe in "one-size-fits-all" batteries. A 6-year-old’s Kindergarten readiness screen is a completely different undertaking than a complex adult Autism/ADHD diagnostic profile. We determine the specific tests needed during your initial consultation so you aren’t paying for irrelevant data, and we provide a Good Faith Estimate (GFE) before you spend a dime and the amount of testing needed will be based on your specific questions and concerns. 

  • Yes. Almost all Health Savings and Flexible Spending Accounts recognize psychological testing, counseling, and consultations as qualified medical expenses. We process your HSA/FSA card just like a standard credit card, allowing you to use pre-tax dollars for your care.

  • A Superbill contains the specific clinical codes your insurance provider needs to see to process a claim. While we provide this automatically at the end of an evaluation, we cannot guarantee reimbursement—that depends entirely on your specific Out-of-Network insurance benefits. We recommend calling your carrier and asking specifically about coverage for "outpatient psychological testing."

  • We understand that comprehensive testing is a significant investment. We generally require a reservation payment to hold your testing date, with the remaining balance due after the consultation. However, we are happy to discuss customized payment schedules if arranged in advance of your first session.

  • At this time, we are not enrolled as providers with Medicaid or Medicare. Our services are strictly private-pay, though we can provide the documentation needed for you to seek reimbursement from private commercial insurance.

We accept your online inquiries 7 days a week and have specific office hours for in-person engagements. Outside of normal office hours you can receive immediate assistance at a local Urgent Care facility or, if necessary, please dial 911 for emergency services.
