Insurance & Reimbursement Information – Helpful Tips
At Mind Mosaic Psychology, we operate as an out-of-network provider and do not file insurance directly. We provide a Superbill (an itemized receipt) that you can submit to your insurance carrier for reimbursement.
Superbills include:
Your provider’s information (name, NPI, address)
Date and type of service
CPT codes (these are the standard codes insurance companies use to identify services)
Fees charged
Here are some tips to help you determine your benefits and navigate the process:
Important: Provider Credentials
When speaking with your insurance company, it is helpful to note our specific clinical designations. These credentials ensure that you receive the most accurate quote for benefits:
LPA (Licensed Psychological Associate): Our clinicians are Licensed Psychological Associates.
HSP-PA (Health Service Provider-Psychological Associate): All our clinicians are certified as Health Service Providers. This certification is critical for insurance companies to recognize our services as eligible "Health Services."
Independent Practice (Sarah Smead): Sarah Smead, HSP-PA, holds the Independent Practice designation in North Carolina. This means she is authorized to practice and bill independently without supervision.

Before You Call Your Insurance
Have both sides of your insurance card handy.
Have the full name, date of birth, and address of the person being evaluated.
Call the number listed for mental health services (sometimes called “behavioral health” or “substance use services”).
What to Ask Your Representative
We recommend calling the "Behavioral Health" number on your card and asking the following:
1. General Benefits
"I am seeking services from an out-of-network Licensed Psychological Associate who is also a Health Service Provider-Psychological Associate (HSP-PA). Do I have out-of-network benefits?"
"What is my out-of-network deductible, and how much has been met?"
2. Coverage for Psychological Testing Codes If seeking an evaluation, ask about these specific codes:
96130 & 96131: Psychological Testing Evaluation Services
96136 & 96137: Psychological Testing Administration & Scoring
3. Coverage for Specific Therapy Codes Ask if the following codes are covered and if they require preauthorization:
90791: Initial Diagnostic Evaluation (Intake)
90834: Individual Psychotherapy (45 minutes)
90837: Individual Psychotherapy (60 minutes)
90846: Family/Parent Psychotherapy (without the patient present)
90847: Family Psychotherapy (with the patient present)
Note on Medical Necessity: Insurance usually only covers services deemed "medically necessary." Testing for academic purposes, giftedness, or certain ADHD screenings may not be covered. We recommend requesting benefits for "general cognitive and behavioral testing.
Payment and Superbills
Payment Policy: For comprehensive testing, full payment is collected at the conclusion of the intake appointment (billed in two equal payments).
Documentation: After the feedback session, we will provide the Superbill for you to submit.
Authorization Support: Our providers are happy to complete any required preauthorization paperwork after your intake appointment to assist you in the process.
