Insurance & Reimbursement Information – Helpful Tips

At Mind Mosaic Psychology, we do not accept insurance directly. However, we provide a Superbill you can submit to your insurance company to request reimbursement. A superbill is an itemized receipt that your psychologist provides after a session or evaluation. It includes:

Your provider’s information (name, NPI, address)
Date and type of service
CPT codes (these are the standard codes insurance companies use to identify services)
Fees charged

Here are some tips to help you determine your benefits and navigate the process:

Before You Call Your Insurance

Have both sides of your insurance card handy.
Have the full name, date of birth, and address of the person being evaluated.
Call the number listed for mental health services (sometimes called “behavioral health” or “substance use services”).

What to Ask About when speaking with your insurance company:

Clarify that you are requesting out-of-network benefits.
Ask about deductibles: How much has been met, and what percentage of the bill would be covered.
Ask about coverage for therapy codes: CPT codes 90847 or 90846.
Ask about coverage for the initial intake: CPT code 90791.
If you are interested in psychological testing, ask about: CPT codes 96130, 96131, 96136, 96137, 96146.

Clarify if preauthorization is required and request any forms.
Ask if specific diagnoses are not covered.

Important: Insurance companies often only cover services deemed medically necessary. Testing for academic purposes, giftedness, or sometimes ADHD may not be covered. Request benefits for general cognitive and behavioral testing.

Payment and Superbills

For comprehensive psychological testing, our policy is that full payment is collected at the conclusion of the intake appointment. This is billed in two equal payments.
After the feedback session at the conclusion of the evaluation, we will provide a Superbill that you can submit to your insurance for potential reimbursement.
At the beginning of the evaluation, after the consultation/intake appointment, our providers are happy to complete any required preauthorization paperwork to assist you.
