Your Client Portal
Accessing the TherapyNotes Client Portal
You can reach the client portal at: https://www.therapyportal.com/p/mindmosaic/
Use the login credentials given to you initially in your welcome email. Once logged in, the main page will have any upcoming appointment information.
In the upper right corner, click DOCUMENTS. Here is where you will find two tabs:
PAPERWORK AND FORMS: These are documents assigned that are just available in your portal in case you need to complete them, or Jen or your clinician have asked you to complete). This may include documents like the following:
Payment Authorization
Intake documents
Privacy Practices
Release of information
A copy of your psychological evaluation
MMP Release of Information Forms:
The release of information form is a document that gives Mind Mosaic Psychology and your clinician permission to share information with and/or recieve information from. this form can be submitted at any time in your client portal or by clicking this link.
CLIENT RECORDS: You may click this link tab to see forms you have completed.
Completed forms can be found under the “client records” tab.

Records are maintained securely for at least 7 years from the date of your last appointment. For clients who are minors, records are kept for at least 7 years or until the client reaches age 21, whichever period is longer. We recommend downloading and saving a copy of your final report to a personal secure drive for your records.

Yes, our client portal utilizes industry-leading encryption to ensure that your personal health information, messages, and documents are completely secure and fully HIPAA compliant.

On the portal login screen, simply click "Forgot Password" to receive a secure reset link via email. If you continue to have trouble, please call or email our front office for assistance.

Yes, finalized evaluation reports, intake forms, and Good Faith Estimates are securely uploaded to your portal for you to review, download, and share with your care team as you see fit.
Schedule an Appointment
We accept your online inquiries 7 days a week and have specific office hours for in-person engagements. Outside of normal office hours you can receive immediate assistance at a local Urgent Care facility or, if necessary, please dial 911 for emergency services.
